Course overview
This inter-professional module will provide professionals from health, education and social care disciplines with essential knowledge and skills to provide holistic, inter-professional care to children and young people with life limiting conditions and their families. Students will be able to explore inter-professional working relationships between health and social care professionals who can support life limited children and young people within a wider children’s palliative care framework. Practitioners will be supported to develop a toolkit of resources that will enable them to provide care to life limited children, young people and their families. This will include discharge planning, caring for the client group across different settings, and taking account of the holistic needs of the child or young person and their family e.g. counselling, bereavement and spiritual support. This will also incorporate systems that support practitioners e.g. clinical supervision, debrief and reflective practice.

Learning Outcomes
Knowledge and Understanding:
- Demonstrate a critical understanding of common chronic illness pathophysiology and the theoretical models and policies that guide professionals in caring for the child/young person with life limiting conditions and their family and how these can be utilised in practice.
Intellectual skills:
- Critically analyse the impact of chronic illness on the individual and the family, friends and the community.
- Design appropriate care based on current evidence base for a child or young person with a life-limiting condition and their family.
Practical Skills:
- Provide holistic care for a child or adolescent with a life-limiting disease and the family, communicating in a developmentally appropriate way.
- Apply evidence based assessment strategies for the child with life limiting conditions.
Transferable skills:
- Working collaboratively with inter-professional teams.

Prerequisites
240 credit points of which no more than 120 credit points at level 5 and no less than 120 at level 6 or equivalent. In exceptional circumstances, if an individual has evidence of recent professional learning they may enter with 60 credits at level 6 or equivalent.
